Thinking without words

So, after reading some things about unsymbolized thinking, I've determined that it may be relatively common, contrary to what some articles discuss.

Someone comes up to you. You have nothing on your mind, yet you're still thinking, maybe planning. Words can't really describe your thoughts. The person proceeds to ask you, "what's on your mind?"
Everything and nothing at once. Something. Can someone comment and tell me if that's common? Do people come up to you and ask what you're thinking, but you can't describe the thought?
